Care Management

ICL’s Pathways to Wellness Health Home program offers care management and coordination for adults and children with serious mental illness, HIV, often multiple chronic conditions who are receiving Medicaid. Care coordinators address each person’s needs in the most comprehensive and integrated way possible by connecting them to healthcare, social services, employment, housing, benefits and entitlements, and other community resources.

Specialized services are geared to children with complex trauma and serious emotional disturbance (SED); a child care coordinator connects them to supportive services and reviews all plans with the family.
