ACT’s mobile treatment and rehabilitation teams are designed for people with serious mental illness that interferes with the ability to function in the community. These are people for whom traditional clinic‐based approaches have not been successful. This recovery‐based model makes services accessible wherever a person may be found, at home, in a shelter, on a street corner.
Multi‐disciplinary ACT teams include a psychiatrist, nurse, social worker, and specialists in substance use and employment as well as peers. Help is available 24/7. The goal is to engage in a sustained relationship that supports recovery.
ICL ACT teams are based in Bushwick, Central Brooklyn, East Brooklyn, Central Bronx and Brooklyn Shelter Teams at our Chapel Street location and the East New York Health Hub.
